Help for Ex-Offenders After Prison
Individuals leaving prison and returning to the community are provided mental health and drug/alcohol treatment and support. Staff monitors and supports people during this transition to reduce the likelihood of repeat criminal behavior and ensure a successful transition back in the community. Recovery services may include assessment, treatment planning, therapy, drug testing, recovery management, medication monitoring, case management and housing assistance. Services are offered in Hartford. Staff speaks English and Spanish.
To Apply
All referrals come directly from the Federal Bureau of Prisons Health Services/US Probation Office and Pretrial.
Individuals referred to the program must:
- Be on probation, parole or transitional supervision
- Be adjudicated and over the age of 15
- Have a completed referral packet and/or treatment service plan
- Submit a signed release form
Additional Details:
Recovery services may include assessment of substance abuse or mental health treatment needs and criminogenic needs; individual and group therapy; supervised drug testing; crisis intervention; treatment planning, care coordination and progress documentation; referral for alternative and/or higher level of care; medication monitoring; psychotherapy; life skills and anger management; recovery management; recovery-oriented vocational services; faith recovery support services; housing assistance; sober housing and intensive case management.
