The Steps to Success approach was developed by the Community Renewal Team (CRT) to help individuals and families who require multiple services, interventions and programs to achieve economic stability. Steps to Success is a holistic service delivery approach designed to help people build a more successful future for themselves and their families.
STS Components
Eight components are built into the CRT Steps to Success delivery approach:
- Intensive case management
- An assessment of the participant’s current situation with the Self Sufficiency Outcome Scale Matrix (OSM)
- A team approach to coordinating services
- A customized service plan for interventions, activities and services (Individualized Service Plan)
- A concentration of resources, programs and services
- Ongoing progress and outcomes reporting
- An internal quality assurance review
- Staff training and feedback
Self Sufficiency Outcome Scale Matrix (OSM)
The OSM interview is conducted during the participant’s initial visit with a CRT case manager. A series of basic questions are asked to identify assets and barriers to economic stability. Questions relate to the individual’s current living and employment situation — income, employment, housing, insurance, childcare, budgeting, and more.
Individual Service Plans
The results of the OSM are used to create an individualized service plan (ISP) tailored to the specific goals and needs of each participant. Interventions, services, contacts and anticipated outcomes are documented in the plan, including timetables and action steps. Progress is monitored regularly. The plan is updated as needed to ensure progress toward goals.
Intensive Case Management
Steps to Success pairs intensive case management with an internal resource team to connect participants to programs and services identified in their service plan. This team approach promotes the sharing of resources across CRT programs and the delivery of concentrated resources.
Quality Assurance (QA)
Monitoring systems are in place to ensure program integrity. File audits are conducted by program management and the QA Department. Staff training and feedback is provided when needed.
